Currently seeking qualified homecare Physical Therapist for placements in all five of New York City’s boroughs.  Therapist reports to Patient’s Nurse Case Manager for Plan of Care, then evaluates/assesses needs of patient, formulates treatment plan for the homecare setting, and provides therapy as defined in plans. Excellent communication skills and team player mentality required, as Therapist must work cooperatively with physicians, case managers, and other skilled professions in the home if applicable. A  high level of creativity and latitude,  in addition to the ability to promptly submit accurate and compliant documentation is also required.

Good, tight geographic clusters available to minimize traveling time. Ability to create own personalized schedule.

MINIMUM QUALIFICATIONS
